Our entire team is dedicated to offering you the finest individualized care possible including outstanding medical and surgical outcomes in a compassionate and caring environment. We believe it is truly a privilege to serve our patients and we are grateful for the opportunity to care for those who entrust us to provide the best possible vision.

Before your visit, you will receive a text message, email or automated phone call reminding you of your appointment. Please make sure we have your cell phone and email address on file. Please respond directly within the message to confirm or to let us know your plans have changed. You will also be asked to check-in prior to your visit and complete a previsit questionnaire regarding insurance and medical information. Please take the time to do this either on your smart phone or on a tablet or computer. This will save time when you arrive for your appointment.

If you request to reschedule or cancel, we will be in touch about new available times.

You should be prepared to spend approximately 1-2 hours in our office, depending on the type of visit you are having. This will allow our doctors and technicians the time they need to complete a thorough examination.

PLEASE BRING THE FOLLOWING ITEMS TO YOUR APPOINTMENT:

• Photo identification
• Insurance cards
• Copay due
• A referral if your insurance company required one from your primary care provider
• Current medication list
• Eyeglasses
• Contact lenses

DILATION: You will most likely have eyedrops instilled in your eyes to dilate them so the doctor can see the entire eye during your exam. We provide temporary sunglasses and most people feel comfortable driving after dilation but if you are not, please be safe and have someone drive you.
